The clinic will be in-person on Wednesday, October 22, 2025. There will be two sessions. The first session will be at 5:00 pm and the second session will be at 6:00 pm in the Student Union in room 261. You can attend either one.

Who will make your medical decisions if you can’t due to illness, injury, or incapacity?
UNC Charlotte Student Legal Services, Inc. (SLS) will host their second healthcare power of attorney clinic on October 22, 2025, at 5:00 pm and 6:00 pm, at the Popp Martin Student Union in room 261, to help students learn how to legally appointment someone as a agent for their POA form.

What to bring with you to the Clinic:
Please bring a valid, unexpired (e.g. driver license ID, state issued ID, or passport) with you to the clinic. Without a valid, unexpired ID, we will not be able to notarize your health care power of attorney on the day of the clinic.
How to Register:
Registration is required and spots are limited. You must complete the intake form before attending the clinic. There is no cost to attend this clinic.

Important Disclaimer: Participation is voluntary, and all information received will be kept strictly confidential. This clinic is only for currently enrolled students who have paid their student activities fee for the Fall 2025 semester. Alumni, staff, faculty, and SLS board members will not be eligible for this clinic. Third parties and family members can’t attend this clinic with their students.
