Who will make your medical decisions if you can’t due to illness, injury, or incapacity?

What is a Healthcare Power of Attorney?
- It is a legal document that allows you to appoint a person, called an agent, to make medical decisions on your behalf if you are unable to do so.
- It is only effective when you are incapacitated, meaning that you cannot make or communicate your own medical decisions.
- It must be signed in front of a notary and two disinterested witnesses.
- Powers of attorney can be revoked.
Why should you consider a Healthcare Power of Attorney?
Planning ahead for potential incapacity with a healthcare power of attorney can help ensure that:
- If you cannot make medical decisions, then those decisions will be made by people you choose and trust.
- Your wishes are followed by those appointed decision makers.
- Most importantly, you have peace of mind knowing that if something should happen, you have a legal document in place.
